Steps to Completing Respirator Fit Test Certification

1. Hazard Evaluation— To determine whether atmospheric contamination exceeds or is likely to exceed the hazard limit, ORS will perform an evaluation on request.

2. Training— Each wearer must receive basic respirator training from ORS.

3. Respirator Selection— Based on the Hazard Evaluation recommendation from PI/ORS, select appropriate respirator that meets all specifications for hazard.

4. Medical Evaluation— A physician or other licensed healthcare professional (PLHCP) is required to determine whether or not a potential respirator wearer is physically able to wear a respirator without adverse health effects. Every worker must be medically evaluated prior to fit testing and before wearing a respirator.

For Chicago campus, schedule with Corporate Health program at Northwestern Medicine. Be prepared to provide a chart string.

For Evanston campus, schedule with NorthShore University HealthSystem OMEGA.

5. Fit Test— After Steps 1-4 are all complete, you can contact ORS to schedule your respirator fit test. We must test you with your individual respirator, so please be sure to bring it with you on the day of the fit test.
